How would you write the equation for tetramminecopper(II)sulfate monohydrate ([Cu(NH3)4]SO4*H2O) being heated?

It will degrade to ammonia, water and CuSO4

If the material was heated in an open test tube would O2 be a reactant?

It doesn't matter. The result will be the same. Ammonia will not oxidize unless one decides to introduce flame and burn it.
